EV: Clearances/Volunteers
At Gateway School District parents/guardians are encouraged to volunteer. The Gateway School District requires all parent volunteers to submit the appropriate paperwork. Upon receiving these clearances, please be sure to provide the building secretary with the original copies so that the forms can be copied and filed. Below is the information necessary for each level of volunteer:
- Assistive Volunteers shall be approved by the building administrator and shall, prior to approval, submit to the principal’s office for review and verification:
1. Assistive Volunteer Application
2. Criminal History Record – Act 34 Clearance (Original)
3. Pennsylvania Child Abuse History – Act 151 Clearance (Original)
(These records will be maintained in the building for a period of two years.)
- Independent Volunteers and Volunteer Coaches/Sponsors shall be approved by the Gateway Board of School Directors prior to providing services to the District. The following forms must be submitted to the Personnel Office prior to starting as a volunteer:
1. Independent/Coach Sponsor Volunteer Application
2. Criminal History Record – Act 34 Clearance (Original)
3. Pennsylvania Child Abuse History – Act 151 Clearance (Original)
4. FBI Clearance – Act 114 (Original)
5. Tuberculosis test within one year
6. Arrest/Conviction Report Certification Form – Acts 24 & 82
