Students in grades 7-12, planning on participating in a spring sport, please remember all online athletic forms must be completed via the parent Skyward portal. All physicals MUST be completed on the PIAA physical form and MUST be completed on or after June 1st of each new school year. If your student has participated in a previous sport this school year, you only need to complete a recertification - no new physical is required. I’ve attached the PIAA physical form and the health history form, which are printable under the online athletic forms in Skyward, for your convenience. All athletic forms for spring sports will need to be completed by Wednesday, March 1st, or your child will not be able to participate.
